Home Care Packages (HCP) enable older Australians to access affordable services to get some help at home.

HCP’s are designed for those with more complex care needs that go beyond what the Commonwealth Home Support Program can provide.

To be eligible for HCP you must be:

  • An older person who needs coordinated services to help you stay at home.
  • A younger person with a disability, dementia or other care needs not met through specialist services.

An Aged Care Assessment will determine your eligibility for HCP and is based on your care needs.

Halo provides a range of services under the HCP Programme including:

  • Personal Care
  • House cleaning
  • Meals and food preparation
  • Help with impairments or continence
  • Nursing
  • Allied Health Services – e.g. Physio, Occupational Therapy, Podiatry, Speech Pathology
  • Home and garden maintenance
  • Transport
  • Social Support
  • In-home Respite to give your carer a break
  • Mobility Aids, equipment and home modifications

You are expected to contribute to the cost of your care if you can afford to.
